Criminal Record Checks 

Criminal Record Checks are required every 2 years as per Leduc Minor Hockey Association policy. All on-ice volunteers will require a criminal record check to be submitted. 

Please submit a letter request to safety@lmha.ab.ca and fill out your letter for your Criminal Record Check (CRC) to have it processed at the RCMP Office.

The CRC will be processed and returned to you while you wait at the RCMP office. You will need two pieces of ID, one must be a photo ID.

Please have all forms back to the Player Safety Director by November 15th. Please have your team manager collect all of the forms for your team and have them submitted as a team package. Drop-off dates will be arranged once the season begins and team staff are in place. Alternatively, you can scan and email the form to safety@lmha.ab.ca

Team staff without CRCs after November 15th will be suspended from all team activity until presented to the Player Safety Director.

RISK MANAGEMENT POLICY

  1. No player shall be allowed to play or practice without being properly registered with Leduc Minor Hockey and properly registered in the Hockey Canada Registry (HCR).
  2. All Coaches, Assistant Coaches and Bench Staff shall be properly registered to the Team through the Hockey Canada Registry (HCR).
  3. All injured players requiring medical attention shall complete an injury report form and not be permitted to resume play without the written approval of a qualified physician for joint, bone, neck, facial, back and head injuries otherwise written parental approval is required.
  4. All coaches must be certified to coach the level at which they are coaching.
  5. All coaches or demonstrators under the age of eighteen (18) years must be in full equipment.
  6. All coaches, guest coaches or demonstrators over the age of eighteen (18) must wear a CSA-approved helmet with a chin strap.
  7. No player, coach or team official shall consume or be under the influence of alcohol, illegal drugs or performance-enhancing drugs at any time within the arena.
  8. "Hazing" or any initiation rite is not tolerated and will be dealt with by the LMHA executive and Hockey Alberta.
  9. All persons transporting players or team officials to or from any team function must carry adequate personal liability insurance (at least $2,000,000).
  10. No team shall be in the dressing room or on the ice without the supervision of a coach or team official.
  11. Players and team officials shall be personally responsible for damage to any facility they play in or utilize.
  12. LMHA will not be responsible for property lost or stolen in the arena.
  13. No players, team officials, or executive members shall make statements to the news media except game descriptions (scores) and details of upcoming games without the direction of the President.
  14. All play will be as per the regulations of Hockey Alberta and CHA.
  15. All executives and team members will not conduct themselves in a matter that causes LMHA legal or financial liability.
  16. All coaches, managers and other team staff are required to complete a Criminal Record Check with Vulnerable Sector Check every 2 years - Before November 15th.    
  17. Cell Phone Policies (NOTE: NO CELL PHONES/CAMERA ARE ALLOWED IN DRESSING ROOMS. NO EXCEPTIONS!!!!
  18. Fighting will not be tolerated off the ice. Anyone involved in fighting, before, during or after a Leduc Minor Hockey event will face disciplinary action.

Health and Safety

All players shall purchase and use their own water bottles. Water bottles are not to be shared. 

No players shall be on the ice without complete and approved equipment for any on-ice function. All equipment must be CSA-approved.  

Goaltenders of all ages must wear a complete set of equipment.

Hygiene and dressing room safety will be promoted following the Hockey Trainers Safety Program.

HELMETS ARE MANDATORY 

ALL COACHES, PLAYERS, GUEST COACHES AND VOLUNTEERS MUST WEAR A CSA APPROVED HELMET WITH THE CHIN STRAP SECURELY FASTENED WHEN PARTICIPATING IN ANY ON-ICE ACTIVITY.

On December 11, 2008, the info bulletin, 08/27 - Mandatory Helmets on Coaches was issued to all members of Hockey Alberta stated: 
 
Effective November 6, 2008, any coach, trainer, assistant coach, guest coach or volunteer who is on the ice or floor (inline hockey) at practice or a similar function must wear a CSA-approved helmet with the chin strap of the helmet securely fastened. 
Beginning January 1, 2009 failure to comply with this policy will result in the following discipline for all Divisions of hockey: 

1st Offence: Written warning

2nd Offence: 2-Game Suspension

3rd Offence: Indefinite Suspension
